The support we provide:

After initial contact with Hearts and Minds Support we will arrange a meeting either at your home or hospital, usually with your social worker present, to discuss you or your family members support needs and how we can best help you.

The support provided by your worker:

Mental Health Support      

The Workers will build a trusting and positive relationships with the client, assess the individual support needs, develop support/goal plans working towards identified outcomes from the Social Workers support plan, case manage and monitor identified risks, liaise with other professionals and carers involved in the persons care, identify services and activities that meet the persons needs and aspirations. 

They will spend time with the client to accompany and support them to carry out their chosen interests, activities, attend appointments and use graded exposure to improve and strengthen the clients confidence and independence.

As our workers become familiar with the clients routines and activities that they enjoy, they can help them to improve their confidence, become less isolated and improve their mental well being and quality of life.
